The Rehab Center at Masonic Pathways is located in the Jack F. Sanders Health Care Center. Priority placement is available for residents in immediate need of therapy and/or skilled nursing services.

Eligibility
A person admitted to the Rehab Center at Masonic Pathways typically:

• requires one or more forms of rehabilitative therapy (physical, occupational, or speech)
• requires 24-hour skilled nursing care
• may require IV therapy, oxygen therapy, or enteral nutrition

The Rehab Center at Masonic Pathways can often admit residents directly from the hospital. Possible admissions are evaluated on a case-by-case basis and bed availability changes frequently. Please contact Masonic Pathways' Admissions Department for the most up-to-date information regarding possible placement.
