Jack F. Sanders Health Care Center Eligibility & Availability

The Jack F. Sanders Health Care Center is a licensed nursing home accepting private payment, long-term care insurance, Medicare, and Medicaid. Extensive rehabilitation services are available on-site. Priority placement is available for residents in immediate need of skilled services.

A person living in the Jack F. Sanders Health Care Center:

• requires 24-hour nursing care
• requires medication distribution
• may require assistance with ambulation and/or transferring
• may require assistance in multiple activities of daily living
• may require IV therapy, oxygen therapy, or enteral nutrition
• may be at risk for wandering

Jack F. Sanders Health Care Center Availability and Waiting List

The Jack F. Sanders Health Care Center can often accept residents in need of Masonic Pathways' Skilled Services Program directly from the hospital. Possible admissions to Masonic Pathways' Skilled Services Program are evaluated on a case-by-case basis and bed availability changes frequently. Please contact Masonic Pathways' Admissions Department for the most up-to-date information regarding possible placement.

Prospective residents who are not in immediate need of Masonic Pathways' Skilled Services Program are encouraged to apply and be placed on a waiting list. When a bed becomes available, those on the list are contacted according to the date their application was received or the last time they declined a unit, whichever date is most recent. Prospective residents may decline residency up to three times before being removed from the waiting list.
